Event Details

  • Do you know what each resident or client in your care home wants?
  • Does he/she want you to attempt resuscitation if they should collapse suddenly?
  • Does he/she want to be transferred to hospital for the last days of their life?
  • Has he/she nominated someone who you should consult with if they are unable to express what they want?
  • Is there a record of each of the above?
  • Does the process of decision-making and record-keeping show compliance with the Mental Capacity Act 2005?

Advance care planning is particularly relevant for supporting people at the end of their lives. This process is recommended in many good practice guides, including the Gold Standards Framework. http://www.goldstandardsframework.nhs.uk/AdvanceCarePlanning.
